feat(questionnaire): integrate revised intake and follow-up monitoring

Add gated TSQ, Russian C-SSRS Recent, durable responses and notifications, diaries, schedules and audited clinical review. Preserve manual assessment when the Russian follow-up source is unavailable. Refs #52
